11 month old pomeranian loss of hair on ears and not gaining any weight?

Really worried, have already taken her to the vet but was told to change diet, but has yet to work!! She weighs a mere 5 pounds!!!! Please if anyone has any HELPFUL suggestions, other that consult a second vet please answer!!!!
